Title: LPN (Home Health)

Location: West Valley (Laveen, Tolleson, Buckeye, Glendale, Peoria) (Flexible on Area)

Rate: $32 - $34 Per Hour

Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate exceptional clinical skills. As an LPN, you will play a crucial role in providing high-quality nursing care to patients in various settings, including hospitals, long-term care facilities, and outpatient clinics. Your ability to work collaboratively with healthcare professionals and deliver compassionate care will be essential in ensuring positive patient outcomes.

Responsibilities

• Provide quality patient care in the patient's home.
• Provide direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Conduct patient assessments, monitor vital signs, and document medical histories accurately.
• Assist with procedures such as IV infusion, catheterization, and wound care.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients and their families on health management, medication administration, and lifestyle modifications.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Participate in discharge planning and coordinate follow-up care for patients transitioning from acute settings.
• Support patients with developmental disabilities, Alzheimer’s disease, dementia care, and other chronic conditions.

Requirements

• Current state licensure as a Licensed Practical Nurse (LPN).
• Experience in critical care, long-term care, assisted living, or hospital settings is preferred.
• Knowledge of ICD-9/ICD-10 coding and medical terminology is advantageous.
• Strong skills in patient assessment, medication administration, phlebotomy, and vital sign monitoring.
• Ability to work effectively with diverse populations including pediatrics and geriatrics.
• Experience in areas such as hospice care, emergency medicine, acute pain management, or outpatient services is a plus.
• Excellent communication skills and the ability to work collaboratively within a healthcare team.
