Position Summary: SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals experiencing or are at risk of homelessness who have serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits. Under the direction of the Director of Behavioral Health Services, the SOAR Case Manager will work in the community and meet individuals experiencing homelessness where they are, including the streets, under bridges, and in abandoned houses. This position requires at least 2 years of experience working directly with individuals experiencing homelessness who have serious mental illness and/or co-occurring substance use disorder; evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness and ability to work in non-traditional settings and unstructured environments.

  • Work with referral sources and community partners to identify applicants through team meetings, outreach, and referrals.
  • Complete interviews with individuals to gather information to complete SSI/SSDI applications.
  • Gather medical records and other information to complete SSI/SSDI applications.
  • Write SOAR Medical Summary Reports for individual applications.
  • Accompany individuals to appointments at the Social Security Administration.
  • Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.
  • Coordinate case management services with partners and help with providing case management services to individuals when needed.
  • Assist the team with administrative tasks as needed.
  • Track all SOAR applications through completion, including appeals as needed.
  • Ensure data entry into OAT, the national SOAR database.
  • Document client engagement through case notes according to established policies and procedures.
  • Participate in staff organizational meetings and treatment planning review meetings.
  • Drive clients to appointments, as needed.
  • Other duties as assigned.

Physical Demands: While performing job duties, an employee is required to stand, walk, and use hands/fingers. May occasionally lift and move up to 25 lbs., clean (sit, bend/twist neck, bend/twist waist, squat, climb, balance, stoop, crouch, reach, handle, kneel, crawl, reach above/below shoulder height, repetitive movements, wipe, sweep, dust, mop). Specific vision abilities required by the position include close vision, color vision.


Work Environment: The noise level of the work environment can range from low to moderate.

Work Schedule

  • Non-Remote Position
  • Full-time: Monday through Friday, 9:00 AM to 5:00 PM
  • Ability to work a flexible work schedule (i.e., evenings, weekends, holidays, and on-call), as needed.
Requirements:
  • Education : Bachelor’s Degree in Mental Health/Social Work or related field preferred; any appropriate combination of education, experience and/or certification will be considered.
  • SOAR Case Manager must be SOAR trained and will attend any trainings/certifications as required by the state. Applications will be reported in the SOAR Online Application Tracking (OAT) Program.
  • Experience: Prior experience and understanding of causes of homelessness. Familiarity with Social Security Administration’s Listings of impairments. Experience with providing outreach and successful engagement to a diverse population, includes working with clients, developing trust, and conducting interviews in non-traditional settings and unstructured environments. Knowledge of Housing First.
  • Language : Proficiency in oral/written English; Spanish preferred; additional languages a plus (i.e., Creole).
  • Skills : Proficiency in computer (Word, Excel, other data collecting programs).
  • Successful completion of Toxicology test, L2 and local background screening, and OIG.
  • Valid FL Driver License with clean driving record and be legally authorized to work in the U.S. on a full-time basis.
