Summary

This position plays a vital role in removing psychosocial barriers that patients face during their treatment journey. As part of a supportive team of social workers, you will be accountable for managing a patient panel for multiple medical providers and leading support groups for the Samaritan Cancer Program.

• JOB SUMMARY/PURPOSE
  • Provide comprehensive assessment, diagnosis, treatment planning, and educational support to patients and their families within a clinic setting, following the Patient Centered Medical Home (PCMH) model to address psychosocial challenges.
• DEPARTMENT DESCRIPTION
  • The Hematology & Oncology teams specialize in diagnosing and treating adult cancer patients utilizing various therapies. We are committed to educating patients about their conditions and providing supportive care throughout their treatment and survivorship.
• EXPERIENCE/EDUCATION/QUALIFICATIONS
  • Licensed Clinical Social Worker (LCSW) in Oregon is required.
  • Training and experience in Cognitive Behavioral Therapy (CBT) techniques for depression, trauma, and anxiety are essential.
  • Experience in a healthcare social work setting is preferred.
  • Bilingual (Spanish) is a plus.
